Is Manual Lymphatic Drainage Safe During Pregnancy?
One of the most meaningful compliments I've ever received in my practice came not from what a client said, but from what she did.
She was a midwife. She understood the body during pregnancy on a clinical level — the hormonal shifts, the circulatory demands, the way fluid accumulates, the ways the body both adapts beautifully and struggles under the weight of growing a human life. And every single month of her pregnancy, she came in for a manual lymphatic drainage session.
She didn't need me to convince her. She already knew.
That image has stayed with me — a woman who spends her professional life caring for pregnant bodies, choosing MLD for her own. I think about her whenever a client asks me whether lymphatic drainage is safe during pregnancy, because the answer, for most healthy pregnancies, is yes — with the right therapist, the right technique, and always with your provider's knowledge and blessing.
Let me walk you through what you need to know.
Why Pregnancy Puts Extra Demand on the Lymphatic System
Pregnancy is one of the most physiologically complex things a human body can do. During pregnancy, hormonal changes and increased blood volume promote fluid retention — particularly in the lower limbs. Peripheral edema is one of the most common manifestations in pregnant women, occurring due to sodium and water retention and changes in circulation related to how the growing uterus affects blood return to the heart.
Hormonal shifts in progesterone, estrogen, HCG, and prolactin also create changes in the permeability of vascular walls, promoting fluid leakage into surrounding tissues. These circulating hormones can create not just swelling, but cramping, varicosities, nerve symptoms like numbness and tingling, and a sense of heaviness and fatigue in the legs and feet. In other words, your lymphatic system is working harder than it ever has — and sometimes it simply needs support.
What Does the Research Say?
This isn't just anecdotal. A study published in Nursing Research and Practice evaluated the effectiveness of manual lymphatic drainage on lower limb edema in pregnant women between their fifth and eighth months. Participants received a one-hour session of leg-draining MLD, with edema measured on both a treatment day and a non-treatment day. The results showed a clear reduction in fluid accumulation in the legs on days when lymphatic drainage was performed. PubMed Central
Research also suggests that MLD may reduce carpal tunnel and nerve-related symptoms during pregnancy by improving microcirculation and removing fluid accumulation that can make nerve sheaths less able to glide freely — a symptom that affects up to 20 percent of pregnant women. Concordiaclinic

The Benefits Pregnant Clients Experience
In my practice here in Greenville, the pregnant clients I work with most commonly come in seeking relief from:
🌿 Leg and ankle swelling — particularly in the second and third trimesters when fluid accumulation is at its peak
🌿 General puffiness and that heavy, weighted feeling in the body
🌿 Fatigue — when the lymphatic system is sluggish, the whole body tends to feel it
🌿 Carpal tunnel symptoms — swelling around the wrists and hands is more common in pregnancy than most people realize
🌿 Nervous system overwhelm — the deeply rhythmic, gentle nature of MLD activates the parasympathetic nervous system, which is exactly the state a pregnant body needs to rest, restore, and prepare
Some practitioners also note that regular MLD sessions during pregnancy may support the body's preparation for labor by promoting relaxation, reducing tension in the pelvic area, and improving overall circulation — which is vital for delivering oxygen and nutrients to the developing baby. So — Is It Safe?
In a healthy pregnancy, manual lymphatic drainage can be safe in any trimester when performed by a trained professional, with the approval of your obstetrician or midwife, and when the abdomen and areas with prominent varicose veins are avoided. Properly applied MLD is superficial and works on subcutaneous tissue — there is no evidence that it causes premature labor when performed with proper technique and precautions.
That said, there are situations where MLD is not appropriate during pregnancy. I will always ask about your health history before we begin, and I will always defer to your provider's guidance. Please let your OB, midwife, or primary care provider know you're considering MLD before booking.
MLD is not advised during pregnancy in cases of:
🌿 Preeclampsia or high blood pressure — conditions affecting circulation require medical supervision before any bodywork
🌿 Active infections or fever
🌿 History of blood clots or deep vein thrombosis
🌿 Unexplained swelling that has not been evaluated by a provider — sudden or severe swelling during pregnancy should always be assessed medically first
🌿 High-risk pregnancies — please consult your care team before booking any bodywork
If you're unsure whether you fall into any of these categories, the answer is always: ask your provider first. That's not a disclaimer — that's genuine care for you and your baby.
What a Session Looks Like for a Pregnant Client
MLD during pregnancy looks a little different from a standard session. Positioning is adjusted throughout — particularly in the third trimester — to keep you and your baby comfortable. In the first trimester, sessions are kept shorter due to increased sensitivity. In the third trimester, positioning is carefully managed to avoid prolonged time on the back. The technique itself remains the same: feather-light, rhythmic, deeply intentional work that moves fluid gently toward your lymph nodes and supports your body's natural clearing processes.
Most clients leave feeling noticeably lighter — in their legs, in their body, and in their nervous system.
